Our Maternity UIF application service include the following: We e-mail/fax all the necessary forms to you as well as our information letter. On commencement of your maternity leave, we submit your application forms to the UIF offices. We follow up on your claim and notify you once it has been approved. After your claim has been approved, we submit the UI-4 form (payment form) on a monthly basis to ensure continuation of your electronic payments.
